Eligibility is determined by financial means.  Eligibility is based on being

between 100% and 200% of federal poverty level.  Click here to see the current numbers.

Patients that have current medicaid insurance do not qualify for this program.

3.  What are the dates of your event?

Feb 11, 2011 will be the DENTAL screening day for all patients.

4.  What are the locations that the event will be held?

All Health Departments in Pinellas County, St Pete College, Johnnie Ruth Clark Clinic, PTEC dental assisting program

2.  How do I sign up for my child to take part?
A financial screening process is necessary.

STEP 2:  TAKE COMPLETED PAPERWORK TO QUALIFIED FINANCIAL SCREENING LOCATION AND THEY WILL SCHEDULE YOUR CHILD AN APPOINTMENT FOR THE FEBRUARY 11, 2011 DENTAL SCREENING/EXAM DAY.
STEP 3: MAKE NOTE OF YOUR APPOINTMENT AND ARRANGE TRANSPORTATION.  YOU WILL ALSO RECEIVE A CONFIRMATION PHONE CALL FROM 211 PINELLAS.
