1.  Is my child eligible for for the Give Kids a Smile Program?

Eligibility is determined by financial means and between ages 4 and 14.  Eligibility is based on being between 100% and 200% of federal poverty level.  Click here to see the current numbers. Patients that  have current medicaid insurance do not qualify for this program.  If you do not qualify, please click here to view county resources to access dental care.

3.  What are the dates of your event?

Feb 8, 2013 will be the DENTAL screening day for all patients.

4.  What are the locations that the event will be held?

All Health Departments in Pinellas County, St Pete College, Johnnie Ruth Clark Clinic, PTEC dental assisting program

2. How do I sign up for my child to take part? A financial screening process is necessary. STEP 1: CLICK HERE TO DOWNLOAD Application for Free Services STEP 2: Complete Paperwork and: take to FINANCIAL SCREENING LOCATION , SCHOOL SOCIAL WORKER and they will schedule an appt and receive paperwork. Mail or drop off paperwork to 501 S Missouri Ave, Clearwater, FL 33756 and we will contact you for an appointment.
